Good Evening, I am looking for advice. My first class medical and special issuance expired in 2017 and I stopped training because life happened. My special issuance is a cardiac valve replacement and I have not had any issues at all. I’m now at a point in my life where I’m able to afford my training again. I was curious if anyone has experience with the process of re-applying for a first class medical w/an expired special issuance. My special issuance letter stat “Not valid after ...”. I sent a copy of my echocardiogram CD to OKC and have my stress test and ame scheduled within the next six weeks.
Does your 2016 SI auth. letter say in paragraph 3, when "this authorization is valid until, __/__/____? If so, what's the date?
01/03/17 It also states it’s not valid for no other class after this date as well. So it will not drop to a 3rd class medical. The entire certification was only valid for 1 year.
Do you need a first class? If not, you held a valid medical after held a medical after July 14, 2006, and had an SI for the valve, so you'd be eligible for basic med.

Provide whatever was on the old letter. AME cannot issue but if your deferred exam is good and the data are good, a certificate shall appear in about 8 weeks, (currently), for nonpsychiatry conditions (only).
Okay, thank you! I sent the letter over to the AME and I’ve also sent a copy of an echocardiogram to OKC. Anything else you recommend doing prior to the exam as far as sending items for medical or should I wait to send the the remaining documents until I see my ame prior to my exam?
How can I give detail advice on a letter I've never seen! Facts not in evidence? There are very few conditions in which an Echo is all that is needed and you're asking me to guess. Where is the cardiology doc's narrative in this picture? If you don't sent it all, you get........noplace.
Just to give an update, I just left the stress test and completed 5 stages and achieved 105% of my maximal heart rate. 191bpm was required and I exceeded the number. I feel great after leaving the appointment and had a full screening immediately after the exam. I have already sent my echocardiogram off to OKC and will complete my faa medical within the week.
