The bacon is cooked first (slowly -- you don't want it too crisp), drained of its excess fat, and tossed into the water along with the other ingredients while the water's still cold; then they are allowed to simmer together.

It could also be done the other way, of course. I don't think there's like any rules about it. It's just soup.

For beef or chicken soup, I usually just barely brown the meat first (it'll still be raw inside) and then let it cook in the stock along with the other ingredients. I cook the bacon more thoroughly before introducing it to get rid of the excess fat more than any other reason.
