On the Basic med what form do I take to the Dr. to fill out?

Go to AOPA's Basic Med page and follow the instructions. You need to be sure that you can qualify, then you do the knowledge course, then follow the rest of the instructions.

Another piece of advice:

You know how little you know about BasicMed? Chances are, unless your doc has done several, he or she knows even less than you do. Educate yourself so you can educate your doc.
