A 747 starts the takeoff without clearance

Tower: “CKS690 LUAW”
CKS690: “LUAW”
Tower: “CKS690 traffic holding in position on the intersecting runway”

Obviously she meant EDV5117 instead of CKS690. She made a small mistake while the Connie crew made a huge one. Kudos to her for owning her part instead of hiding behind their part.
 
Those kind of mistakes are a lot easier to make when you fly the long duty time back side of the clock schedules that they do.

She didn't make a mistake. The subtitles are wrong.
 
Takeoffs when told to line up and wait? Happens everywhere. Heck, I did it myself in a T-6 as a student...in formation.
Or as I often utter in debrief: "you leaned so far forward you bent yourself over...." :rofl:

A lot of these pilot deviations fall in the category of "many are guilty, some get punished for it". Life is about timing and luck. Those who fancy themselves immune from it are the ones you have to watch out for.
