I do not meet the vision requirement in one eye for a second class medical. I have amblyopia since birth and my vision is stable. I submitted the form 8500-7 to my ophthalmologist but he can not complete the form. He does not do all of the tests required but he had an optometrist in his office that can compete the tests and form. Will this be acceptable to the FAA or does the form have to be completed by an ophthalmologist? Thank you.

I don't know but hopefully someone can answer your question. If not, what does your examiner Doc say? AOPA has a medical number you can call. Or you can call the FAA medical branch. Good luck.

In obtaining special eye evaluations in respect to the airman medical certification program, reports from an
eye specialist are acceptable when the condition being evaluated relates to a determination of visual acuity,
refractive error, or mechanical function of the eye. The FAA Form 8500-
7, Report of Eye Evaluation, is a form
that is designed for use by either optometrists or ophthalmologists.

Found on page 186:

The vision in my left eye cannot be corrected to meet the minimum requirements. I had to take a medical flight test that involved spotting other aircraft in the pattern, doing a simulated engine out descent from directly above the runway to landing, and then back to base. The whole flight lasted about 20 minutes.

Basically, if you can demonstrate that you can see well enough to fly safely, and that the condition is stable and not deteriorating, you'll get your medical via a special issuance. (SI)
