Question on reporting an issue

A few loops of the roll servo cable on a GFC500 has come off the pulley.
Cable is crossed over itself in a few places, looks pretty ugly.

If the cable gets hung up the ailerons travel will be limited or frozen.

Shop is going to fix it but I doubt they’ll report it.
I’ll ask them to, but if they don’t I think I have an obligation to report it.
What’s the proper tool/system? Is it just a service difficulty report?

SDRs are reqired only when there is " serious failure, malfunction, or defect of an article." If it's been explainable by prior bad maintenance or improper installation, then it's optional.
 
To me ; key is why did the cables do this?

Most pulley/cable systems have keepers of some sort to

reduce” excitement”.

Are they missing or another design flaw?


I have the predecessors of SDRs ( inspection AIDS and Airworthiness Alerts )

going back many years.

When I encounter a “ new to me aircraft” I find it worthwhile

to research what folks have found with the type over the years.

Sometimes the issue has become an AD but often the problem

still exists.

Often thought of marketing a file by type.
 
Figured it out today. The inner servo cable clamp has slipped on the aileron cable.
You can see in the first picture the clamp is ridding up on the pulley cage.
On the outboard side there’s a lot of slack, hence the cable slapping I heard when moving the ailerons.
 
At one time there was an AD on certain AP.

It was really hard to tell which ones because the units were often rebranded

as Autocontrol etc.

The 1969 gave no details on identifying the culprits so you had to pay

to mfg $50 for a copy of Service Letter!

The issue was involving loose clamps similar to what you encountered.

It required replacing the clamp screws with bolts which I believe you have.

I’m sure some are still unmodified. ( Comanches)

Writing it up could save lives.

SDRs are voluntary for some people but regulatory required for others like Part 145, 135, 121, etc. Regardless, anybody can submit a report which is encouraged by the FAA and there is an online site specifically for that. The reporting criteria for those entities required to submit reports varies and is dictated by their approved operating specifications.
Is it just a service difficulty report?
SDR is the collective term for a number of different report styles: MDR, MRR, MMIR, etc.
