There's a number of free online OCR. Try several and pick the best. If it's small and only one then trying several should be easy.
 
I don't know about software, but if it's one small document it may be easier and cheaper and faster to just hire someone to type it in.
 
Scan it with your phone, upload to your computer. Save as PDF. Then open in Word. Should work.
If not, open as JPG, save in Word.
If not, you've only wasted a few minutes.
 
There are a few apps for that.... some are free to try.
