DA vs DH Confusion

It's says RA NA.
Where?

It's supposed to, but it doesn't. It says "RA 100". The only place "NA" appears on the FAA approach plate is in reference to simultaneous approaches (Ctrl+F the PDF).

That's disturbing. Do you happen to know how long the mistake has existed?
Don't have a clue. I had made my original comment based on the Jeppesen chart. Then, when I saw the posting of the FAA chart it was obvious one was wrong. So, I looked up the source, which anyone can do on the NDBR tab at the FAA's IFP Gateway. Moral of the story: The department in the FAA that makes the FAA approach charts is not the same department that designs and issues the 8260 forms. In a strict sense they are a chart maker, just like Jeppesen and LIDO. The difference with FAA charts is that they must conform to government charting specifications. None of this prevents errors.
