Googlefu. Look at Google Images for Whirli-gig cub. There was a link to it on Amazon, where the manufacturer information was readily available. It took about 30 seconds (time to kill while on a conference call).

Yes. They said this propeller has an AD on it and would cost $15K to make it airworthy again.
