Ground loop or gear up.

Then there was the time I backed a fire truck 1 inch too far. Left a dent exactly the shape of the end of the squirt.

You typically start a little lower.

Maybe the seat or gearshift safety switch was bypassed. Two locals in the past 10 years died trying to start their tractors while in gear. Another lost a leg to a bush hog, stopped against the barn or he would have lost more.
