Reporting question

If I'm going in multiple times for titer/booster/titer/booster (until I get a positive titer), can I put that on one line, or do the titers and boosters need to be reported as two different line items? Testing and Vaccination isn't quite the same. To complicate it, it's a different nurse every time I go in, so I may have to list them all separately anyway... and I haven't paid enough attention to get their names. argh!

You need to report all visits to health professionals. However, "Multiple visits to one health professional for the same condition may be aggregated on one line." While it may be a different nurse each time, I'll be it's the same office, and if you put it down as one line entry with the supervising physician's name, I'm sure the FAA will be perfectly happy.

Thanks Ron. I thought you had to put the name of whomever you saw... Just putting the supervising physicians name will simplify that greatly.
