Replacement SODA for deficient color vision

HI all, I recieved a SODA waiver in 1995 and have misplaced it. Can anyone give me guidance on the process of requesting a replacement?

I'd contact the local FSDO. I have the same SODA. I scanned it and stored it. But I am sure the FSDO will get you straight.

Thanks for your response. Just contacted FSDO and they put me in touch with the right contact in Oklahoma City, they are requesting my records and sending me a duplicate. I had been using a folded up paper of a passed Farnsworth Lantern test since 2007 which I always presented to the doctor at my medicals. I was asked to take it again yesterday which I passed but don't care for the hassle being I have a SODA.
