Question about filling out Dr. visits

How should I put down a Dr. visit for a mole check/skin exam? Should I put down "routine exam"? or "mole check" and the DNP wasn't concerned about my moles. I want to put down accurate info so as to not get a letter from the FAA asking for more info. Thanks for the time.

I'd ask the doctor how he listed the visit on his record and just use that.
