Ophthalmologist in San Francisco Bay Area familiar with form 8500-14 for glaucoma?

Hello all,

I am hoping to find an ophthalmologist in the San Francisco Bay area who might be familiar with running tests and filling out the form 8500-14 for glaucoma.

My current ophthalmologist is refusing to fill the from out and I was hoping I could get a lead on an ophthalmologist who is familiar with the process and has done it before for other pilots.

If you can't get the 8400-14 filled out buy a board certified ophthalmologist, get his latest office visit record. They are usually equivalent. Modern ophthalmology finally, has typewritten records!
