My fiance was convicted of DUI in 2009 and his pilots certification taken at that time. What process does he need to do to obtain his pilots license again? The FAA was notified at the time of conviction.

The FAA does not revoke pilot certificates merely for a single reported DUI. Medical certificates, perhaps, depending on the history and severity of the event, but not pilot certificates. For your fiancé to have had his pilot certificate revoked, there would have to be more history than a single DUI conviction.

In any event, if one's pilot certificate is revoked, the process to obtain a new pilot certificate is as follows:
  1. Wait at least one year after the revocation.
  2. Obtain the instructor endorsement for the written test for an entry-level pilot certificate (Sport, Recreational, or Private) written test.
  3. Take the entry level written.
  4. Get at least three hours of flight training from a CFI and obtain a recommendation for the practical test for that entry-level certificate.
  5. Take the practical test for that entry level certificate.
  6. Repeat the endorsement/testing cycle for each higher level certificate/rating previously held.
All previous logged experience may be used to meet the experience requirements for the new certificates and ratings, but all the recommendations and testing must be re-accomplished. In this case, the biggest problem might be getting the medical certificate, since it would have to have been a major history of alcohol-related issues to result in a pilot certificate revocation, and that history could make it nearly impossible to obtain a new medical certificate without many thousands of dollars of evaluations and medical tests and ten years or more after sobriety is achieved. Bruce can fill you in on the details of the medical evaluation process.

Also, note that the Administrator has the power to see that someone who's had their ticket revoked never again gets another pilot certificate, but this is a power very rarely exercised.

Did he complete his 61.15 report in time? Or was he late in the report?

Did he have a prior DUI?
There's just not enough information here....

From the medical branch, has he been to rehab?
Is there evidence of recovery activities?
Has he been abstinent- and can it be proven?
The list of quex is very long.....
