I don't know how many of your are aware of the ASRS Callback website, but I subscribe to it and it does make for some interesting reading. This month's issue includes a helicopter that inadvertently had a tail rotor blade installed backwards, and a training flight that ran out of fuel.

In case you didn't know, every month the NASA ASRS program collects reports that pilots like us make, and picks a few particular reports of interest and puts them out in a newsletter. All identifying information of course is scrubbed.
