I work in a hospital and was exposed to whooping cough, I got a call from one of the Dr. and said we should all go on a prophylactic antibiotic. I used the antibiotic and never developed any symptoms. How would you report the healthcare visit? When I just got a call and said to go to the pharmacy and pick up your meds. Never diagnosed and never got an exam.

Doesn't sound like you had a visit at all. If you're still on the meds when you go for your medical, just report it. That's going to be a yawner. If you want to report it as a visit just put down the Dr's name and "occupational exposure to pertusis." Again, it's going to be a yawner by the AME.
