Dave, I used the AOPA TurboMedical and took the form with me to the doctor's office all filled out for my own reference.

Dave, ditto what Diana said above - use Turbo Medical...it saves from session to session, lets you start again where you left off and prints the form for you to take with you...I like it...I like it a lot!

D I swear I saw B's post then none ...til tonight,.... when I saw G's point out - otherwise I would have missed it (ABOVE B's). Is something going on with the board? I notice NO Recent Activity and New Posts are missing the down arrow "go to first unread message" so I have to read them all again.
