Have reviewed the Medexpress user guide online where it states list approximate date for surgeries. Have read through my paper medical records I asked for from my
Eye physician. Only one eye surgery is listed with an exact date, I had the exact same surgery done a second time but there is no mention of the date in the physical paperwork I have; nor does the doctors office have it on their electronic records when I called today. Possibly due to them telling me they switched over from paper to electronic around that time. I know the surgeries were performed within less than 3 months of each other, over 7 years ago. On the medexpress form should I put within so many months? Since that’s all the information I have on when.
