Mechanical failure or pilot error?

Looks like they were doing a maintenance check where they perform a "sim retract". This is done to check for leaks (usually) and the gear has to be pinned.

Looks like someone forgot to put the safety pin in the nose gear.

Youch, I wonder if the hydraulics could pick it back up.
 
I assume the gear is power up/down and one can use gravity in the event of a failure of multiple backups?

I'd guess the hydraulics wouldn't make anywhere near the force needed to lift the nose.. the gear leg is really light in comparison.
