Loose screw

Most likely a spinner screw, came out and was batted by the prop blade. ice is worse danger, that's why you never see a window directly in line with the prop.
 
Or.....when I was in the Air Force, we had a prop blade come off a kc 97 refueler, at 20,000 feet. It hit the aircraft, it decompressed , went into a flat spin, training crew on board, unable to jump due to centrifical force, 10 died when it hit and blew up. Lots of in air accidents at this time including b 47's , several carrying atomic weapons at the time. Lots of accidents!
