Read that, and think about it the next time you're on one of those A340s doing a "flex" takeoff (the leisurely taxi-into-the-sky method).
 
That helpful diagram doesn't quite explain what the DC-8 captain talking about in regards to the distances he cleared the 767.

The point is the DC-8 was at a 10 degree angle of attack as it climbed so it's tail was lower than then it's cockpit. That's the 75 feet vs. 45 feet distance.

An animation of the DC-8 passing over the 767 might have made that clearer.
