When disclosing a previously omitted physician appointment, how is that done? Do you just add it to block 19 for previous 3 year healthcare provider visits or do you attach a separate paper or something else
 
Ask your AME. Unless it was of a significant cardiac, mental health etc visit, report it at your next renewal.
