Not applicable to me (at least at this time), just curious.

You have a medical scheduled and the day before you go, you get some "normal" injury, or illness, or something - make up your own scenario for this question. You decide to go to the appointment anyway.

Like, let's say you broke your leg, or sprained your ankle, or something like that, and are walking with crutches. Clearly you can't fly a plane at that time. But, it's a "normal" injury that should probably heal just fine and does so thousands of times a day. Does the AME say "um, come back when it's healed", or can they proceed with the exam?

Maybe you just threw your back out that morning and it's uncomfortable to walk/sit/stand. Or some other everyday, minor (or major but "normal") injury/illness. You should probably just reschedule the appointment but you don't - maybe for some external reason (like you have a job offer that will go away if you don't have a medical by today. Or you're going out of the country). Something like that.

How are these things generally handled?

Cancel the appointment and reschedule after you are substantially healed. The AME can't sign you off if you are physically impaired at the time of the exam. They don't "know" that you will heal. As with most things, there can of course be shades of gray on something like a mild ankle twist that you took some ibuprofen for and can still get around just fine, but showing up on crutches or unable to show reasonable normal movement due to a pulled back is a pretty hard sell to the AME.
