Hospitilizations question on MedXpress

I was admitted to a hospital 13 years ago for a cellulitis infection, I went to the ER and was admitted under observation and treated with IV antibiotics and was subsequently sent home after 2 days.

Unfortunately I don't have any records from that event like the treating physicians name, or even the Hospitals current name or address. I was working out of town and everything was handled by my employer for me, I never saw a bill or much of anything really. The company that handled everything for me has been out of business for 11 years now.

I want to be honest on my form but I don't know how to answer answer the questions with any accuracy.

im no medical expert or lawyer, but I would just put down 2007 admission for cellulitis infection. thats it. the doctor will probably as you about it and make a note and thats that. on mine i reported an admission 1968 for a hernia repair. the doctor asked, "any complications or follow up?" told him no, he noted it and now i just check yes, PRNC.
