My CRM instructor from the class I took earlier this years was the accident investigator on this one. We got to really dig into the reports and hear his views. BTW the Apache was cracked in half. If they had not 'landed' when they did the thing would have fallen into two pieces in just a few minutes.
The warrant Officer who was the IP in this aircraft was giving an orientation flight to the newly minted Lt. He, the WO, retired as a result of this accident. The type of flying though is something that the Army does a lot. The white path that you see is actually a pathway for the helos to follow at Fort Knox to train them in nap of the earth techniques.
