I had a laparoscopic hernia repair last August. Will I need any documentation at my next physical this fall?

Yes. Any and all surgeries have to be documented every medical, not just the next one.

Turned out just fine, no problems at all. I know I have to report it, but do I need any paperwork from my doctor.

Nope, just the date of the surgery and that is was successful. That's what I report anyway. I don't tell them about my lobotomy.

"Manifest full recovery from surgery".....in AME's box #60. You won't need a thing.
