What's the usual culprit here? You hear a lot about "stuck valve partial power loss" scenarios. Dumb luck or are there some "go-to" causes?

Both times was on the same rental with a high time engine. First time they replaced a jug hoping to get to TBO, after the second one they gave in and got it overhauled.
