Going to Basic Med from 3rd class with SI

Depends entirely on the doctor performing the exam. Because my GP did my Basic Med, he knew about the SI (afterall, he wrote the supporting letters for the SI when I had the Class 3) and all I needed was to reference the date and cause. Remember, the FAA never sees the Basic Med form, just you and the doctor. But you must have the paperwork should the FAA wish to see it in case of some event such as an accident.
 
Thanks for your reply. My GP has all the info from the specialist so I guess I will just say this in the application form. I never realized that the FAA did not see the Basic Med form. It looks like the FAA is largely out of the picture when one uses Basic.
