Getting a new SI after being on basic med

I was flying on basic before I grounded myself for feeling crappy. Went to my cardiologist and it turns out my stent that I previously had an SI for has failed and they’re going to have to repair it. When I’m fixed do I have to get a 3rd class with an SI again?
