A friend of mine forgot to check the box have you ever been diagnosed with allergies in your life. He had a few allergic reactions as a kid but they went away. Not seasonal type or stuffy nose or anything common like that. Just some bizarre episodes as a child to unknown stimuli that just went away which is why he forgot.

Is that going to be a problem?

A friend of mine asked his AME a question about something similar. The doc's, with no hesitation, was, "No, don't put that on there. There is no reason to, and the only way anyone would ever know was if you told them. Of course I would deny ever telling you that."

I really depends on the allergy. In my case I definitely list a couple allergies because I carry an emergency card with me all the time noting I'm allergic to penicillin and sulfa-based drugs. Those can be life-threatening to some people.
