Excellent story that highlights how organizational accidents exist, thrive, and can be stopped in areas besides our cockpits. In this case, it's the hospital.

I work for a large IT outsourcer and this is one of my pet projects - managing the risks and nature of organizational accidents. Our bottom line directly relates to our ability to quickly diagnose, fix, and solve difficult technical problems, but it also relates to our ability to do so flawlessly and repeatedly. The aviation world provides a great model, in some cases, for organizational safety programs. People find these programs obtuse, buerocratic, and stifling until they see a crisis that was completely averted.
