I last had a Class III issued in 2004. I took a few lessons and had to stop due to other commitments.

Is there a way to get a copy of what I submitted on my application? I know what I would probably have listed as prior issues/operations/etc, but some of the dates and details for some of those operations and issues elude me now.

NO, you don't want to make a FOIA request. It will be denied.
You can try a privacy act, but that probably won't work much btter.

You need to contact

Librarian, Medical Records, PO Box 26200 Oklahoma City OK 73125

Works perfectly. Got mine in about 10 days for $2.

Ask for your full record, not just medicals. There's sometimes some interesting stuff in there....

I'll try the Librarian first and see what happens. Thanks!

"Previously reported, No change" has worked for me for over 20 years. Not a chance in hell I'd remember everything or would care to write it every time.

I've had a lot of changes since 2004. Could I do a blanket statement like "previously reported, with the following changes"?

Previous things were broken bones, trips to the emergency room for various bumps and bruises along the way. Nothing that would be a continuing problem - it's just that I can't remember all of the dates and details.

Changes are more lingering, but nothing that should impact my medical. I can get details for all of the medical issues from the date of my initial medical.

They will bill you post hoc for the cost of certified mail to your door. If it going to exceed $25.00 they will inform you.
