I need HELP! I am up to three forms filled out none correct and now the badge lady helped me by sending me another one. I may finally be on track to get silver badge #1000000 if I keep up this rate.

seriously dude, the paperwork is not that hard. the badge lady should be able to help you get it right. did you ever get your OO to sign what he needed to sign?
