Yep, need to hear the tapes before deciding who was wrong.
My dad was dinged by Vancouver (Canada) ATC for departing w/o a t-o clearance. This is a decent sized airport. Once the tapes were heard by the supervisor, he was phoned back and offered an apology & told the controller received additional training.

Start @19:30. The incident is between the Kaleo (caravan) and HAL155. I didn't even hear any read back instructions from the Kaleo plane.

19:30-HA155 is cleared for takeoff on runway 2
20:50-Caravan is told to start a left turn, they are not cleared for takeoff.

This like is Clearance Deliver, Ground, Tower, and Approach. Has more chatter but some of it is missing due to multiple frequencies.

When I hear blue donut I think of Breaking Bad. There was a donut shop in Albuquerque that put large blue (the color of meth in the show) sugar sprinkles on their donuts in honor of the show.
