Hello
I am pretty early in the process for getting my medical and filling out MedXpress, I got all of the medical records i could find for recent doctor visits, however the only concerning one i have is relating to vision.
I can get each eye correctable to 20/20 with glasses, however I have an issue with having monocular vision. I know I can do a SODA later on and be ok, however the question and main hurdle is with the form itself.
For the record I have US citizenship now, but was born abroad and had eye surgery in early childhood for a lazy eye. This is related to being able to see only out of one eye at a time. I don't want to omit anything or get in trouble later on, but the eye surgery was in a place its very difficult/impossible for me to get records from, and i am sure there are notes of it by optometrists. Is it possible to just give an approximate date/location or am I just going to be stuck and denied a medical without an actual record?
Thanks

Should be no problem with a 3rd class. Regardless of records, would need a SODA for 2nd or 1st. Should be no problem since you see 20/20 in each eye.

Unless there is something significant, you do not generally need to provide medical records with your exam. The approximate date and location of the surgery will be sufficient, as the concern will likely only be for the result and your current status. But since this is your first exam, you should schedule a consultation with an AME, before you have the actual exam.

Do not finish that medxpress.

Get to a board certified MD ophthalmologist (not OD optometrist, go to the real deal), and get:
FAA's form 85007 filled out and signed, dated.
Humphrey 24-2 visual field maps done. If the good eye's visual field is normal then apply.

You will get an initial denial but then write a letter asking for a medical checkride. You'll get teh auth for same and when you accompish solo, exercies the privilege and take the medical vision check ride.

The waiver is forever.
