A believe the place to get that is from a DPE or from a FSDO.

The original "I don't know it all" of aviation.
When I got my first medical cert, my situation had the AME deferring to OKC. But since I had all of my required documentation and sorted in easy to deal with groups by condition, I had a fairly fast issuance and recieved the medical certificate by mail within 5 weeks.
But they didn't include the student certificate.
The AME directed me to the local FSDO next to KAFW. Set an appointment, met with a very helpful inspector who verified the needed information, and had the student certificate typed up on the spot.

I got just a SPC from a DPE because I called an AME and he said "I dont know how to do that" haha.

I got mine from the FSDO at Sun'n Fun this year. Hopefully I'll be able to use it before it expires.
