    What makes it worse, as with the majority of aviation accidents, is that it could have been avoided. Regardless of crew error there are multiple instances in the report that indicate that plane should've never taken off that day.

    True, but a single engine failure on takeoff in a 4 engine airplane would not have been fatal if anyone on that flight deck had been paying attention.

    This reminds me of the C-5 crash at Dover a few years ago when the pilot flying pulled the wrong engine back and no one else in the cockpit noticed.

    Going to pick a nit here. The C-5 crew at Dover actually identified and pulled the correct engine back. Then at some point they brought all the throttles to idle, and when they eventually went to add power, they advanced the throttles on two good engines and the shutdown engine, leaving the third, good running engine at idle.
