Approved locally, then OKC denies

If an AME approves a medical that may be viewed as questionable, how long until OKC would over rule the issuance?
 
Ostensibly, they're supposed to review things within 60 days, but frankly, the FAA never acts that quickly. There's so much weaseling in the regs, that the FAA really can deny you at any time. The catch-all is "The federal air surgeon determines that public safety would be endangered by the holder's exercise of airman privileges."
