I'm no spring chicken, but I'm not old either (or so I think). Like many I have a medical history, I've answered all the questions to the best of my ability and have provided comments to all the have I ever...."yes" items under #18. Towards the bottom of the paged under Part 3 it lists "General Explanations Pertaining to Medical History". It's not clear to me what that area is supposed to be used for.
When answering yes to any of the items in 18 is the comments section only for the AME to fill out or the applicant? Do my explanations go into the general area towards the bottom or directly next to the yes items.
I just want to make sure I'm reporting the facts in the right place.
