Back [a long time ago] we were making an FMEA for a flight test program, where we'd wired in an Easy Button (the ones Staples sold where you pushed it and it said "That was easy").

One failure mode (didn't make it into the final document) was "Easy button continues to say 'That was easy' on infinite loop" resulting failure "Pilot goes insane and flies into mountain"
