Stress Assessment

Anyone know where I can find that stress assessment that asks whether certain things have happened in one's life over the course of a year, and assigns a value based on the answers?

I tried googling it, but I guess I didn't hit the write combination of terms. I kept using things like "assessment," "annual," and so forth that threw it off; and didn't use "events," which should have been an obvious one.

Let's see..
2002, daughter graduates high school, goes 3000 miles away to college and I retire from one career and start another.
2006, daughter graduates college (dad stops paying college bills)
2008, daughter gets married (dad stops subsidizing daughter totally)
2009, daughter and new husband buy house.. but they are still 3000 miles away
