Routine Visit

When completing box 19 of form 8500-8 "Visits to health professionals within last 3 years" does anyone list the reason as "routine visit"? If so under what circumstances would it be appropriate? Some people I know suggest using this rather than giving an explicit reason.

I have put down "annual physical" with no adverse effects.

I put down "Annual Check up, prescription renewal", I've already listed my Cholestrol med.

"Routine" implies a regular examination for no other reason than the passage of time. If there are symptoms which led to the visit, it's not "routine," and then the actual reason (and any diagnosis arising therefrom) should be listed. Likewise, a condition-driven follow-up exam (like a cholestorol level check for someone with hypercholesterol) should be so listed (e.g., "periodic cholesterol check, no significant findings").
