Reporting PRK to FAA

Hello everyone

I had a successful PRK procedure done 6 months a go, tomorrow Im seeing my ophthalmologist I will have him fill out Form 8500-7 should this include separate status report?
Where do I send these documents is there a subdivision in FAA that takes care of this?

Yup. What you need actually isn't an 8500-7, but is a typewritten (the won't accept writing from opthalmologists) saying that your vision is stable and you have no ongoing issues.

This CAN be written on an 8500-7 but, guess what? Most opthalmologists SCRIBBLE. So I'd just ask for a letter.

Then mail it to AAM 331, PO Box 26200, Ok City OK 72135-0080 and do it CERTIFIED RETURN RECEIPT. The agency is currently in a bad way, CERTIFIED for any and everything. Yes it's a pain to go to the Post Office, but it's less of a pain than letting months go by and no knowing if they can find it.....
