My 3rd class medical expires this month so I called up my AME's office to schedule an appointment. I was told that if I am taking any medications that I needed to bring a statement from my doctor stating that I am stable on the med(s) and the underlying condition(s) is under control. She said it was a new FAA requirement and the AME has to keep the statement of file. This was the first I have heard of this additional requirement. It seems odd that the FAA is adding requirements at the same time legislation is trying to eliminate the 3rd class medical altogether.

Depends on the medication. Technically, many of them require a status report but it seems to be widely ignored by AMEs for a lot of the tamer stuff (the approved antihistamies, statins, etc...) who just ask if you've had any side effects and make their own notation.
