When did your last medical expire?
If after July 14, 2006, look into Basic Med. Exam by your normal doctor or even a doc-in-a-box (similar to DOT CDL physical) and then do an online course for free.
Otherwise, here is the search page for FAA examiners of all types. You are looking for an AME (Aviation Medical Examiner).
It is HIGHLY recommended to first fill out a paper form and schedule a CONSULATION with the AME. DO NOT fill out the online form. And if you do, DO NOT GIVE THE NUMBER TO ANYONE AT THE AME OFFICE. Once the AME activates the online form, they must either Issue or Defer your medical. And you want to know where you stand before you do anything official. With a consultation first, you can see where you stand, and is there are any issues, you can work to having all the paperwork for the FAA BEFORE you are denied and have to fight them.
Here is why, if you do a consultation and find something that will give you trouble getting your medical, you can fly Light Sport or Gliders without a medical. If your medical is denied, you cannot drop back to Light Sport or Gliders as you have a known issue.
