Letter from my MD

Requesting letter from my MD re my post op status and looking for guidence as to how he should address it. To Whom it May concern?

A few years back the doctor I was seeing for post-op followup addressed the letter to me.

It doesn't really matter. Address it to you, or the FAA. or better yet address it to no one. Have him format it more like a standard discharge summary. Your doctor should know what those are.

Normally if the FAA wants something, they'll tell you exactly what they want. Be sure your doctor gives them exactly what they ask for - no more, and no less!.
