Let's say you're at Matha's Vineyard and fly 30-40 miles offshore and come back. Would you have to go through immigrations when you came back?
 
Short answer is NO, if you go fly in other airspace but do not land (and are in continuous radar coverage), you don't have to clear customs. Canadian overflight is a good example of this.

However, if you suddenly appear on ATC's radar from international airspace, and you haven't been coordinating with them properly, you will be in for an interesting experience.

Heck, I've been in "international airspace" on the way to and from Key West. Customs aren't involved but you do have to file an ADIZ flight plan and squawk a discrete code.
