2 parts...

1st Part -- Had a bad enough dizzy spell back in 2002 that made me go to the Dr. He said it was BPPV and referred me to a specialist that performed the Epley maneuver on me. Naturally, before I go to see the specialist, the episode was over, He did the maneuver anyway to show me for future instances. Since then, I get the episodes now and then but not bad enough to keep me home or not drive. I understand that the FAA wants 12 months between episodes but I wonder if anyone has had an episodes while flying IFR and how did it turn out? Although it only has happened in the middle of the night while sleeping, I don't know if that will always be the case.

2nd part -- I looked back at all my medical forms for prev Class 3's and I never annotated that I had this BPPV. I think I just forgot all about it since it was more or less and inconvenience with moving my head and I just dealt with it. I haven't flown since 2007 and now want to go back up but am concerned. 12 months between episodes is more or less a no fly ever sentence for me. Now, I have no problem giving it up if I have to in the interest of safety but I wonder if I do go for a medical, how do I report something I forgot to for that time ago? I want to do the right thing. Suggestions?

How would I go about filling out the top section where it says "Have you ever" and I didn't mention this in prior Class 3 applications? Like I said in my post, I had totally forgotten all about it. Do they compare past applications to a current one and possible ask why it wasn't on prev applications and now on this one?
 
I don't have a solution for you. But if it were my airman I would make the comment in box 60 and be done with it. Then it has been reported....just not by you, but by me.

I was in the process of getting all my dates in order for medexpress and came across my referral for the Physical Therapist for the BPPV episode I mentioned earlier. I was mistaken. The date of the last episode was mid April, 2012 and not 2007 as I prev thought. Will this have a cause for my AME to defer my Class 3 I plan on going for in Jan 2017 or could he issue based on last episode being years ago. I have no reports from the Dr saying I am no longer experiencing BPPV, I just stopped having episodes..

I suspect that's him saying you go through that twice, and then it's "Previously Reported No Change" after that, but the wording isn't completely clear.
 
You sure did omit, but nobody is going to care about something that long ago. The AME mentions it in his commentary, it's swallowed by the agency and it's never revisited again, and becomes "Prevously reported". The agency will send you al letter of "eligibility, warning", most probably.
