Generally, the outline is:
(1) a letter from the neurologist that describes you capabilities with the meds and
(2) then you show up to the AME's without the med, and he evaluates you for capability w/o the med.
IF 1 & 2 are satisfactory--special issuance with the letter.
IF 1 is satisfactory and 2 is not, no issuance as FAA cannot control if you took your meds the morning of the accident.
